FOR 74 DOST EMPLOYEES / RECIPIENTS (3 DOSES PER PERSON) INCLUSIVE OF ADMINISTRATION OF VACCINES BY DOCTORS/NURSES AS PER GIVEN SCHEDULE.

 

 

SCHEDULE OF ADMINISTRATION OF HPV VACCINES:
1ST DOSE – DECEMBER 2013 (2ND OR 3RD WEEK)
2ND DOSE – FEBRUARY 2014 (2ND OR 3RD WEEK)
3RD DOSE JUNE 2014 (2ND OR 3RD WEEK)

 

DELIVERY OF VACCINES:
BY LOT – 74 VIALS/PCS PER GIVEN SCHEDULE; TO BE DELIVERED DURING ACTUAL DAY OF VACCINATION

 

TERMS OF PAYMENT: 30 DAYS
SEND BILL AFTER THE ADMINISTRATION OF THE VACCINES TO THE RECIPIENTS AS PER GIVEN SCHEDULE

Qualified bidders should submit their quotation formnot later than December 20, 2013 (9:00 AM). Failure to strictly comply with the deadline and general conditions shall automatically disqualify the bidder/s from the bidding process.
